Our Philosophy

At Foundations Senior Living Placement Advisor, we believe that every person deserves to live with dignity and respect. It is our philosophy to provide the help and knowledge to find the best care that is individualized, comprehensive and compassionate.

What Are assisted living homes?

Assisted living and nursing homes offer a range of care levels tailored to residents' physical, cognitive, and medical needs, from minimal assistance to full support, with specialized memory care for individuals with dementia.

TYPES OF LEVELS OF CARE

Level 1: Minimal or Independent Care

Level 1: Minimal or Independent Care

Level 1: Minimal or Independent Care

  • For residents who are largely independent but may need occasional reminders or minimal support with ADLs.
  • Services may include medication reminders, basic housekeeping, or assistance with complex tasks

Level 2: Moderate Assistance

Level 1: Minimal or Independent Care

Level 1: Minimal or Independent Care

  • Residents need hands-on help or supervision for several ADLs, such as bathing, dressing, grooming, or medication management.
  • Support is designed to help maintain independence while addressing evolving care needs.

Level 3: High or Ongoing Assistance

Level 1: Minimal or Independent Care

Level 3: High or Ongoing Assistance

  • Requires consistent support with most ADLs and potentially multiple caregivers.
  • Social interaction and structured activities are included to support wellbeing. Residents at this level may also show mild cognitive decline.

Level 4: Full Assistance

Level 4: Full Assistance

Level 3: High or Ongoing Assistance

  • Residents need near-constant care, including help with almost all ADLs, feeding, mobility, and hygiene.
  • Staff provide intensive, hands-on support, with care focused on maintaining residents' remaining capabilities.

Level 5: Memory Care

Level 4: Full Assistance

Level 5: Memory Care

  • Specialized care for residents with dementia or cognitive impairments, often in secure units.
  • Staff are trained in dementia care techniques, providing supervision, behavioral support, and tailored activities to ensure safety and stimulation.

What We Consider

Assessment and Care Planning

Facilities determine the appropriate level of care through formal assessments, examining:

  • Ability to perform ADLs and instrumental ADLs (IADLs)
  • Cognitive status and memory function
  • Medical conditions and mobility
  • Mental health and social engagement
  • Family involvement and care preferences

Understanding how to pay for senior living

What are payment options for care homes and senior living communities?

Understanding how to pay for senior living is one of the biggest concerns families have, and we're here to guide you.

Private Pay

Most families begin with private funds such as savings, retirement income, pensions, or social security.

Long-term care insurance

If your loved one has a long-term care policy, it may cover a significant portion of assisted living or memory care costs.

Medicaid (for qualified individuals)

Some communities accept Medicaid or offer Medicaid waiver programs, depending on eligibility and state guidelines.
